			<content:encoded><![CDATA[<p>A 74 year-old female with borderline renal function and a history of spinal stenosis has an epidural catheter in place. She is post-op day 2 from a colon surgery for cancer.</p>
<p>This morning, she was diagnosed with a PE, and was given 1mg/kg of enoxaparin. Six hours after the lovenox, the nurse calls the acute pain service. Apparently the epidural catheter has become disconnected in the last 3 hours (the disconnect was unwitnessed). The tips were found lying on the bed near the patient&#8217;s urinal.</p>
<p>How would you manage this situation?</p>

			<content:encoded><![CDATA[<p>To cap off <a href="http://www.anest.ufl.edu/gator-rap/2009/06/falling-numbers-case-discussion-for-the-week-of-june-8-2009/">this week&#8217;s case discussion</a>, we&#8217;d like to return to the last teaser of a question: </p>
<p>What does ondansetron have to do with hypotension when the Bezold-Jarisch  reflex is involved? </p>
<p><a href="http://journals.lww.com/rapm/Abstract/2008/07000/Ondansetron_Given_Intravenously_Attenuates.8.aspx">This article from RA&#038;PM</a>describes ondansetron&#8217;s possible role in hypotension, the Bezold-Jarisch reflex, and neuraxial anesthesia. Importantly, this review concerns spinal, and not epidural, anesthesia. However the mechanism described may also function similarly following hypotension and bradycardia seen with epidurals. </p>

<p>For the pharmacologists and structural biologists among us, <a href="http://en.wikipedia.org/wiki/5-HT3_receptor_antagonist:drug_discovery_and_development">here</a> is a very cool Wikipedia article on the discovery and drug development of seratonin receptor antagonists.</p>
<p>Kudos to Drs. Lucas and Koyyalamundi for picking up on the involvement of seratonin in this reflex through their comments this week.</p>

<blockquote><p>AN international drug company made a hit list of doctors who had to be &#8220;neutralised&#8221; or discredited because they criticised the anti-arthritis drug the pharmaceutical giant produced.</p>
<p>Staff at US company Merck &amp;Co emailed each other about the list of doctors &#8211; mainly researchers and academics &#8211; who had been negative about the drug Vioxx or Merck and a recommended course of action.</p>
<p>The email, which came out in the Federal Court in Melbourne yesterday as part of a class action against the drug company, included the words &#8220;neutralise&#8221;, &#8220;neutralised&#8221; or &#8220;discredit&#8221; against some of the doctors&#8217; names.</p>
<p>It is also alleged the company used intimidation tactics against critical researchers, including dropping hints it would stop funding to institutions and claims it interfered with academic appointments.</p>
<p>&#8220;We may need to seek them out and destroy them where they live,&#8221; a Merck employee wrote, according to an email excerpt read to the court by Julian Burnside QC, acting for the plaintiff.</p></blockquote>
